Medical Testing
If you are scheduled for medical testing (CT Scan, Ultrasound,
Mammogram, etc.) to be done at Cavalier County Memorial Hospital
(CCMH), you will need to register before having your test.
Please check in at the front office of the hospital 10-15
minutes before your scheduled test. If this is your first visit
to our facility, please bring your insurance card(s) with you.
If you have been here before, we will verify all information
that we have on file. If there have been any changes to your
information since your last visit, please notify our staff.
We need to know of any changes to your address, insurance information,
emergency contact information or changes to your marital status.
If you are scheduled for Mammography or Bone Density tests,
there are forms located on this web site that you may print
out and have completed when you arrive. This will save time
